
Research Associates (RA) provide important information that informs future health care for all of us. That's why LifeX RAs must pass a health Assessment Questionnaire as a prerequisite for employment or receiving health insurance benefits. Please review the questionaire so that you can answer the questions for each person you want to have health insurance through your group plan.
Part of becoming a LifeX Research Associate is choosing a health insurance plan. There are five plan groups, each with its own deductible options, plan benefits, copays, and premiums. These are available below for your information. If you choose to apply for employment, you'll select a plan to start the application process. You can download the selected plan brochure at that time.
*Deductible amounts shown are for the employee only. Other deductible amounts will be shown for employee and spouse, employee and children, or employee and family. All employees and those they insure must meet health assessment questions. These plans are underwritten by Benefit Logistics Captive Insurance Co, Inc NAIC # 17633 and not by PHCS or Cigna Licensee.
