Steps for the application

 ​Before Starting Your Application

1. Proceed with applying to become a LifeX Research Associate if you can answer NO to all Health Questions for yourself and any family members you want to include. If you answered YES to any health question, thank you for your interest but you will not qualify for this position.

2. Choose the health insurance policy you'd like to enroll in.​ Write down the name so it's easy for you to select. It will be the first thing you will need to do when applying.

3. Determine if you want to add vision and dental plans. You will see those options on the application page where you choose your health plan.

• Steps When You Go to the Application Page

1. When you click the Apply Now Button below, you will be taken to a page with Affordacare Insurance at the top. You'll see a list of all available health insurance policies. Select the one you have determined is the best for your situation. Review the details; check the rate to verify the premium; download the brochure if you'd like. Click ENROLL for that policy. That will take you to the beginning of the application. 

2. You'll be taken to a screen where you will add your name and other basic information. You'll also add the names of others you want to be covered on your insurance options. Make sure the health insurance you selected is correct, and add a dental and vision plan if you want that coverage. You'll also see the total monthly premium. 

3. After you provide this information, you will be asked to enter your basic address and contact information. Fill that out completely. Then you will see a section to add information for each person listed. Those names will be on the right side of the screen. Click the first name and fill out the information. When you save each one, the blank form will return, and you check the next person until all are complete. 

4. You will be asked the lifestyle questions next. These are about you, not your entire family.  Be sure you reviewed them earlier so you can answer them easily. Remember, you are not being asked to give up your current job. This is as little as fifteen minutes of your time each month. These questions are not disqualifying, but they must be answered accurately.

5. Next, you'll answer the health questions for yourself and each family member as a group you added at the beginning of the application process. Please understand that each person you are enrolling must answer NO to all health-related questions. If you have any questions that you must answer yes to for the group, you will not be eligible for this position.
   DO NOT proceed with the application. 

6. Finally, you'll enter your payment information for your first month's premium. Future premiums will be made on the 15th of the month using the same method you provide. To be covered on the first of next month, you must apply, pay the first premium, and be approved by the 22nd of the current month.
